As part of completing your child’s enrollment at Al Nebras School, we kindly ask you to submit the following forms at your earliest convenience:
- Student Medical Report and Consent Form
- Authorization Form for Pick-up
- Photo Consent Form
Submitting these forms ensures a smooth and complete enrollment process for your child.

Important Reminder to Parents

=> If your child has a chronic illness (asthma, diabetes, epilepsy, severe allergies, G6PD, heart conditions, etc.), please provide the school with a formal medical diagnosis or medical certificate from your child’s doctor.

=> If your child requires daily or emergency medication (inhaler, EpiPen, insulin, etc.), please submit it in its original packaging with a prescription label and complete a Medication Authorization Form with the school nurse.

=> For safety reasons, students should not carry their own medication unless approved (e.g., inhalers for older children).

The authorized person must always bring his/her Qatar ID when coming to school for security purposes. A copy of the authorized person’s QID must also be submitted along with this signed form to the School Administration.

Photography and Videography Consent
